Chronic Disease Management (CDM) & Enhanced Primary Care (EPC) Plans at Bodyfix for Physiotherapy & Exercise Physiology Services

At Bodyfix, we are committed to providing comprehensive support for individuals managing chronic health conditions. Under the Chronic Disease Management (CDM) Plan or Enhanced Primary Care (EPC) Plan.

Eligible patients may receive Medicare rebates for physiotherapy and exercise physiology services aimed at supporting symptom management, improving physical function, and promoting long-term wellbeing.

Our team works closely with each client to address their specific health needs, providing guidance on managing conditions through targeted exercises, pain relief strategies, and sustainable lifestyle changes. For eligible patients, some services are bulk-billed, while others may involve a gap payment, ensuring you receive high-quality care tailored to your needs.

The Chronic Disease Management (CDM) or Enhanced Primary Care (EPC) Plan is a Medicare initiative that provides rebates for allied health services for individuals with chronic conditions. Frequently Asked Questions

Eligibility for the CDM/EPC Plan is determined by your GP. Generally, you may qualify if you have a chronic condition (> 6 weeks duration) that requires ongoing management. Your GP will refer you if they believe you would benefit from physiotherapy or exercise physiology support. Ask your GP at your next visit if you are eligible and can they complete a Care Plan.

Medicare covers up to five allied health sessions per calendar year under the CDM/EPC Plan. These sessions can be divided among different services, such as physiotherapy and exercise physiology, based on your needs and referral.

Some CDM/EPC services may be bulk-billed, while others may require a gap payment. Our team will inform you of any gap fees in advance, so there are no surprises.

The CDM/EPC Plan is designed to support a wide range of chronic conditions, including arthritis, diabetes, heart disease, respiratory conditions, musculoskeletal issues, and more. We customise our services to address each client’s specific health needs.

Once you have used your five Medicare-rebated sessions, you are still welcome to continue treatment. Our team can discuss private payment options, private health rebates, or alternative funding solutions to ensure you receive ongoing support if needed.

No, the CDM/EPC Plan is not means-tested. Medicare rebates for these services are available to all eligible patients with a chronic condition, regardless of income or financial status.

You are eligible for another set of five Medicare-rebated sessions at the start of each calendar year, regardless of when your Care Plan was initially written by your GP. This means you can access new sessions every January, providing ongoing support for managing your chronic condition.

Medicare provides a total of five allied health sessions per calendar year under the CDM/EPC Plan, which must be shared across all allied health providers. If you exceed these five sessions, you will not receive a Medicare rebate for additional sessions. It’s important to keep track of your sessions to avoid any unexpected out-of-pocket expenses. Unfortunately, we have no control over this limit, so please be mindful of the number of sessions used with each provider.
